To schedule an appointment as a first-time client, please fill out and submit the form at the bottom of this page. You will receive a phone call and/or an email after you complete the form to confirm your request and schedule a firm time and date for an appointment. Session payment must be made online in advance of your scheduled appointment. Each session is one hour, though extended 1.5 hour sessions can be requested (please allow for an extra 30 minutes on your first session - no additional charge). Additional time may be requested during scheduling, and is subject to availability. At this time, we are unable to accept insurance directly. However, please feel free to inquire about how we can assist with insurance reimbursement paperwork.

* RUNNING LATE – All appointments are subject to a 10-minute policy, as sessions cannot begin after 10 minutes into the scheduled time. If you have not arrived within 10 minutes of your scheduled time, your session will need to be rescheduled. You may be granted a one-time application of your payment to the rescheduled appointment. After this one-time exemption, late arrivals will forfeit the original payment and will be charged for future appointments.

* CANCELLATION POLICY – If you are unable to attend your scheduled appointment, a cancellation will need to be made by email or by phone at least 24 hours prior to your session. Emergency situations will be handled on a case-by-case basis.

* New Client Paperwork forms can be accessed by clicking the link below. This is an online form and should take approximately 5-10 minutes to complete. The forms will be submitted online with an electronic signature. No printing necessary!
